  3. Complete the forms requiring signatures. There are four forms that must be completed before submitting your application - the Indemnity and Consent Form, the Photo/Video Release Form, the Medical Authorization Form, and the Medical Information Form. These forms will require the electronic signatures of a parent/guardian. Please note: these forms must be completed for your application to be processed.
  4. Complete the Essay Questions.
  5. Upload a Photo of yourself. These photos will be used to make a participant board for the cafeteria to help learn each other's names!
  6. Complete the payment information, and submit your payment for the program. Please note: if payment is not received by May 31, 2017, you will lose your spot in the program.
  7. Submit your application.
  8. You will receive a response via email within two weeks from the time we receive the signed forms and payment confirming your acceptance into the program.
  9. After you have been accepted into the program, you will be sent one additional form to complete: a Travel Information Form. Please complete this form promptly - we cannot have you in the program without having this form completed.
  10. After receiving your Travel Information, we will send you an email to let you know it has been processed. Then, we will direct you to our website where you can find the Participant Information Package. Please review the package to learn more about the program, and what you'll need to bring.
